The Notice of Privacy Practices describes how we may use and disclose your PHI in accordance with applicable law, including the Health Insurance Portability and Accountability Act (“HIPAA”), regulations promulgated under HIPAA, including the HIPAA Privacy and Security Rules, and the NASW Code of Ethics. It also describes your rights regarding gaining access to and controlling your PHI.

A good faith estimate shows your provider's expected charges for items or services. Because the good faith estimate is based on information known at the time your provider creates the estimate, it won’t include any unknown or unexpected costs that may be added during your treatment.
